Project Performance Reports allow agencies and funders to understand and improve project performance and, through that, system performance. Provider agencies may request Project Performance Reports for their projects. Each provider agency may request reports twice in a quarter. Because of that limitation, we encourage provider agencies to identify all HMIS project IDs they want and include them in the requests. Funders may request reports for projects that they fund and do so once per quarter.
Provider agencies will receive reports that include both aggregate and client-level data. Funders will receive only aggregate data. If a funder wishes to receive additional data, that organization should submit a Custom Data Request to Access Chicago’s HMIS Data.
Requirements for Requesting Organizations
- The person submitting the request must be an Agency Technical Administrator or have permission from All Chicago to make requests by following the instructions below in 1b. For provider agencies, an ATA should be the default person making requests so that all relevant projects are requested.
- If your agency does not have an ATA, please contact helpdesk@allchicago.org.
- If you would like permission to request reports for your agency, please provide an explanation for your request and email that to projectreports@allchicago.org. If possible, please cc your ATA(s) on that email.
- The requesting organization must be an organizational member of the Chicago Continuum of Care (CoC) with required dues payments made. Questions about CoC membership should be submitted to CoCprograms@allchicago.org.
- The requesting organization must have a current Agency Participation Agreement with Chicago’s HMIS lead agency, All Chicago. Questions about the APA should be submitted to helpdesk@allchicago.org.
Additional Requirements for Funders
- Funders must demonstrate that they fund the projects for which they request performance data. Before submitting the request, please email evidence of funding and the requested HMIS project IDs to projectreports@allchicago.org. All Chicago will reply to confirm.
- At the time of making the request, funders must contact the provider agency to inform them that their Project Performance Reports have been requested and inform them of the time period and HMIS project IDs requested. Please cc projectreports@allchicago.org when providers are contacted.

This guide provides detailed information for each metric on the Project Performance Report, including a description of the metric, the universe of data it covers, and the types of projects to which it applies. Data Use Statement
The data in the Project Performance Reports is processed and formatted in alignment with each metric’s methodology as described in the Project Performance Reports Methodology Guide. Users should carefully review this methodology to understand the content and construction of the metrics. This data only provides the project’s performance measure related to each identified metric. The CoC affirms the validity of the data only as delivered in alignment with the stated methodology. Further analysis, modification or combination of the data by the user is prohibited. Any questions should be submitted to projectreports@allchicago.org.
All rules and guidelines that apply to handling HMIS data apply to the Project Performance Reports. Agencies receiving the reports should review their Chicago HMIS Agency Participation Agreement (APA). For providers, whose reports include client data, that data should be treated with the same level of care and confidentiality as all client data. If an agency wishes to share this report with any other organization, such as a funder, they must do so only for the purposes outlined in the privacy policy and ensure that the organization receiving the report also has a signed Chicago HMIS APA.
